Hultquist Place - 96-Bed Assisted Living Program (ALP) Providing LHCSA Services

Position Summary:

Hultquist Place is seeking an experienced and compassionate Director of Patient Services (RN) to lead our Clinical Services Department. Serving within an Assisted Living Program (ALP) providing LHCSA services, this leadership role ensures high-quality resident care through strong clinical oversight, person-centered practices, regulatory compliance, and support of resident independence, choice, dignity, and safety. This position directly supervises the Wellness team and ensures excellence in assessment, care planning, communication, documentation, and staff development.

Essential Job Duties:

• Lead and manage all Clinical Services operations within the ALP/LHCSA care model
• Supervise LPNs, Med Techs, PCAs, HHAs, and clinical support staff
• Conduct and oversee resident assessments (including UAS) and care plan development
• Ensure compliance with NYSDOH LHCSA requirements, audits, documentation, infection control, incident reporting, and QA/PI standards
• Monitor medication management practices and overall nursing care delivery
• Provide training, skills check-offs, and competency evaluations for PCAs/HHAs
• Maintain accurate documentation in EHR systems
• Serve as a resource for residents and families; resolve concerns quickly and professionally
• Collaborate with physicians, pharmacies, labs, and home health partners
• Participate in Administrator-on-Call rotation

Qualifications/Experience:

• Active New York State RN license required
• BSN preferred
• 3-5+ years of clinical experience in assisted living, personal care, long-term care, SNF, home health, or similar environment preferred
• 1-3+ years of supervisory or care management experience strongly preferred

Knowledge/Skills :

• Strong knowledge of UAS, care planning, and NYSDOH regulatory requirements preferred
• Strong leadership, coaching, and team-building abilities
• Excellent communication, assessment, documentation, and organizational skills
• Proficiency with EHR systems and basic computer applications
